West Dunbartonshire Community Health and Care Partnership (CHCP) is a strategic partnership between West Dunbartonshire Council and NHS Greater Glasgow and Clyde . It brings together both NHS and Local Authority responsibilities for community-based health and social care services within a single, integrated structure.
The CHCP is one of the first partnerships in Scotland where the
Council and NHS work together to deliver the best community-based
health and social care services.
